Notice

1. New Applications
All submissions for new applications should be made atleast five (5) weeks before the scheduled meetings with all documents indicated on the application form.

2. Continuing review applications
Continuing review applications should be made at least three (3) months before the permit expiry date. Please submit two copies of the continuing review application form, the protocol and consent forms and the annual report of the study. For any changes that need to made, please fill in an amendment form.

3. Amendments and close-outs
Amendments and close-outs can be made anytime during the course of the study. Please submit two copies of the amendment application form attach a copy of tracked changes and a clean copy of the protocol and consent forms
